I-140 Checklist for Foreign Professionals

The Employment-Based Immigrant visa application is made on INS Form I-140. The information and documentation needed for the preparation and processing of the I-140 visa application includes the following:

1. Copy of Passport
2. Copy of I-94 (front and back)
3. Copy of prior nonimmigrant visa Approval Notice (Form I-797)
4. Current resume including your employment history for the past six years (names, addresses, and exact dates of employment)
5. Original college/university degrees and transcripts or notarized copies
6. Most recent W-2’s
7. Most recent pay stub with your employer
8. Foreign address
9. Present address
10. Day and evening telephone numbers
11. Social Security Number
12. Exact dates of unauthorized employment
13. Exact dates of unemployment in the U.S.
14. Copy of Credentials Evaluation (if applicable)
15. Immigration information concerning your spouse and/or children
